We accept referrals from dentists across Greater Manchester for implants, oral surgery, endodontics, orthodontics and imaging. Send the details below and we will contact your patient within one working day. Your patient returns to you for their routine care.
How it works
You send the referral
Fill in the form below. Attach radiographs, photographs or a scan if you have them. It takes a few minutes and you do not need an account.
We contact your patient
We get in touch with your patient within one working day to arrange their assessment. You are copied in so you know it has been picked up.
You get a report
We write to you after the assessment, and again when treatment is finished, so the patient's notes with you stay complete.
What we accept
If what your patient needs is not on this list, call us and we will tell you honestly whether we are the right practice for it.
Single units, bridges and full arch, including All on X
Surgical extractions, wisdom teeth and apicectomy
Primary and retreatment, including molars
Invisalign and fixed appliances
Composite bonding, veneers and smile design
Crowns, bridges, dentures and full mouth rehabilitation
Complex crown, bridge and denture work, seen by a specialist prosthodontist
OPG and CBCT, with the report sent back to you
Send a referral
Everything marked as required is needed before we can contact your patient. Anything you cannot fill in, leave blank and we will follow it up with you.
Please only send us a patient's details once you have their consent to do so. We hold referral information as part of the patient's clinical record and handle it under our privacy policy. If a patient needs to be seen urgently, call us rather than using this form.
What you get back
A referral should not mean losing sight of your patient. You stay their dentist, and we keep you informed at every stage.
- A written report after the assessment appointment, with the plan and costs
- A copy of any radiographs or scans we take
- A discharge summary when treatment is finished
- Your patient comes back to you for their routine care
